Inconceivable difficulty was introduced in Gears of War 4's Rise of the Horde update. In Campaign, players have even less health than on Insane difficulty, but enemies also have less health, and weapons and crates drop less ammunition, making the Campaign more survival-based. In Horde, players have less health and enemies have more health, making it a direct step up from Insane. Inconceivable difficulty is unlocked by completing the Campaign on Insane, and 50 consecutive waves of Horde on Insane.
